Position Description:

Employees in this classification are responsible for providing social services to patients and their families by assisting them with personal and environmental difficulties which predispose illness or interfere with obtaining maximum benefits from medical care. Persons in this position work closely with physicians and nursing staff to further their understanding of significant social and emotional factors underlying patient's health problem. They also serve as a resource referral service in advising patients and their families about available community resources.

Major/Essential Functions:

  • Participates in Departmental Performance Improvement which will satisfy both institutional and contractual requirements.

  • Employee is responsible for convening service planning team meetings and completing annual renewal and developing IPC.

  • Employee is responsible for completing face-to-face quarterly reviews.

  • Responsible for communicating with Direct Service Agency and Individual/Family.

  • Maintain current information on services and products needed by persons with disabilities and be available to conduct presentations as needed.

  • Provides phone coverage.

  • Collaborate with collateral agencies and providers to ensure compliance with Individual Service Plan.

  • Conduct home visits as needed.

  • Be on call on a rotating basis and counsel with clients and provide crisis intervention and advocate as needed.

  • Obtain all necessary authorization to provide services.

  • Perform CLASS billing monthly.

  • Assist with planning, coordination and follow up on WTCMP special projects and provide CLASS training to new employees.

  • Performs all other related duties, within the scope of practice, on an as needed basis or as required by Supervisor.

Preferred Qualifications:

  • Individual casework practice, crisis intervention, information and referral, case management and interviewing.

  • Experience with working with persons with disabilities.

  • Must have degree and licensed or eligible to be licensed. Individual will obtain social work license.

Pay Statement:
