Atlantic Health System Nurse Navigator in Morristown, New Jersey

Responsible for serving as the first reliable point of contact for Patients, Families and Providers.

Principal Accountabilities:

  1. Serves as patient and family advocate.

  2. Coordinates the scheduling of appointments to expedite the plan of care and to promote continuity of care.

  3. Schedules, re-schedules, and cancels appointments for the appropriate outpatient service.

  4. Assists in education of the patient/family to understand disease process and care map.

  5. Assists in answering patient calls, initiate call backs and follow-up to meet the needs of the patient.

  6. Assists with communication to internal departments and external contacts for insurance verification and authorization process.

  7. Supports a smooth transition of patients from outside referrals to internal referrals for appropriate treatment.

  8. Performs other related duties as assigned.

PEDIATRIC ONCOLOGY HEMATOLOGY NURSE NAVIGATOR JOB DESCRIPTION

Overview: Goryeb Children’s Hospital, a part of the Atlantic Health System, is currently seeking a full time Pediatric Hematology Nurse Navigator for the pediatric hematology-oncology unit (The Valerie Center).

Responsibilities:

· Provide continued education and support for all newly diagnosed pediatric hematology patients and their families.

· Coordination of care – ensuring quality patient care, so the family can focus on their child, the patient. This includes but is not limited to planning and scheduling imaging, assessments, labs, consults, medication management, appointments, and family meetings as needed.

· Enhance communication between the family patient/family with the interdisciplinary and interdepartmental teams.

· Point of contact for all referrals, school-nurse letters, and bleeding disorder education to schools/school nurses, handling of lab specimens and send-outs, and collaborating facilities as needed.

· Setting up home care/home health services for patients with bleeding disorders.

· Will maintain a professional role that will continue to adapt and incorporate new evidence-based practice methods to better support patient and family needs.

· Assist APNs with medication prior authorizations.

· Will assist with miscellaneous tasks that arise as needed within the Valerie Center to support safe and quality patient care, as well as workflow.

· Will manage/update hematology diagnosis patient visits.

· Program development for hematology diagnoses

· Develop patient education materials, resource binders, and transition program with adult team for chronic hematology patients.

License/Education/Qualification:

· Current NJ RN Licensed

· BSN

· Must have 3+ years pediatric acute care/hematology/oncology experience.

· CPHON preferred or willing to become certified within 6 months of hire
